There was a time when business apprenticeships were extremely popular. No one knows exactly why they fell out of favor. Today, they have returned, and are more popular than ever. In an age when there is so much uncertainty and so many career options, employers and job seekers are connecting in order to find the best matches. Expect this trend to continue well into the future, as the true benefits are once again being realised.

Employers waste quite a lot of money in hiring employees that quickly discover that they would rather be employed elsewhere. Employees may also experience a waste of their time and job seeking effort when they accept a position that they find dissatisfying. Each of these problems can be completely eliminated by making use of apprenticeship situations. These “trial” jobs are very insightful for everyone involved, and neither is under any obligation to continue to working relationship if they do not wish to.

Business apprenticeships can be used on your resume if you wish. These working relationships are recognized as valuable experience, and are considered a wise move on the part of a job seeker. Of course there is always the chance that the apprentice will offer you a full time working position within their company. This happens quite frequently, and is the best possible outcome if you find the work enjoyable.

I have 35 years and a month ago I suffered a stroke. Maintain a healthy lifestyle, exercise regularly and eat well. What steps I can take to make sure this never happens again?

Response from Dr. Kevin M. Barrett, Neurology, Mayo Clinic, Jacksonville, Florida, USA:

In order to achieve reduce the risk of another stroke in someone who already suffered one, it is essential to understand the possible cause for the former, because that will guide future prevention tactics.

A stroke occurs when blood supply to part of the brain is interrupted or greatly reduced, depriving oxygen and nutrients to brain tissue and causing neuronal death. There are several common causes for a stroke. A frequent cause is narrowing of the neck arteries that carry blood to the brain (carotid arteries) due to the presence of fatty deposits. On the other hand, a tear or a break in the wall of a blood vessel, heart disease and blood clotting disorders may also lead to stroke, especially people like you who still does not meet 45 years. If any part of the reasons mentioned in stroke treatment and prevention should be directed to that issue.

However, in about 35 percent of people who have suffered a stroke can not establish the cause (cryptogenic stroke). If that is the case, then your goal should be to strive together with your family doctor or your neurologist to find risk factors for stroke that you would change in order, to the extent possible, reduce their risk in these areas.

Some risk factors for stroke can not be changed, for example, sex, ethnicity, age and family medical history, all of which participates in any way at risk for stroke. In addition, men are more likely to suffer strokes than women, Asians and African Americans are also at greater risk. After 55 years, each new decade doubles the risk for stroke.

The impact of focus on modifiable risk factors for stroke can be great because it is possible that several treatable conditions alter this risk significantly. People who suffer from hypertension, for example, or have high cholesterol or uncontrolled diabetes are at higher risk for stroke. If you have any of this, talk to your doctor to develop an effective treatment plan. In case you have elevated low-density lipoprotein (LDL or “bad”), take a statin could reduce not only cholesterol but also your risk for stroke.

Besides controlling any underlying conditions, it also helps to choose the lifestyle. You mention that you exercise regularly and eats healthy. Both are important because exercise can lower blood pressure, increase the level of high density lipoprotein (HDL or “good”) and improve the health of blood vessels and heart. It can also help you lose weight, control diabetes and reduce stress. A good goal for exercise is to participate in moderate activity for at least 30 minutes most days of the week.

A diet rich in fruits and vegetables with low saturated fat and cholesterol may reduce the risk of stroke and help you maintain a healthy weight. This is important because excess weight contributes to other risk factors for stroke such as hypertension, cardiovascular disease and diabetes. Other changes in lifestyle to prevent a stroke you can do is to stop smoking and keep your alcohol intake to less than one or two drinks daily.

Similarly, it may also be appropriate for you to take preventive medications, depending on your medical history. Antiplatelet drugs such as aspirin, or aspirin combined with extended-release dipyridamole (Aggrenox) and clopidogrel (Plavix) can make the blood less likely to clot. Anticoagulants such as heparin and warfarin, may serve to prevent a stroke if you have heart problems or blood clotting disorder.

The point is that you can take steps to reduce the risk of having another stroke, but the specific measures that would be appropriate for you depends largely on your medical history. Talk to your doctor about stroke prevention approach that best fits your personal situation.

Unlike refined flour, whole grain cereals contain fiber and nutrients that help control diseases ranging from blood pressure and type 2 diabetes to heart problems and stroke. The benefits do not end there. Discover the thousand and one reasons why whole grains and you must become the best of friends.

Whole grains have been part of the diet of different cultures since ancient times. Over time and with the advancement of societies, they were leaving way for the refined flours. Today we use and consume much more flour and grits without the shell, which gives them a soft and delicate flavor. But what we gain in flavor lost profits. How wise were our ancestors who ate mostly whole grains and their products! It is ironic that those who did not have nutritionists and scientific studies on their behalf, knew exactly what they are most suited to your health.

The Whole Grains Council of the United States lists all the benefits we provide whole grains:
- Reduce the risk of cardiovascular attack by 30 to 36%
- Reduce the risk of type 2 diabetes by 21 to 30%
- Reduce the risk of cardiovascular disease by 25 to 28%
- Help maintain a healthy weight

But if this were not enough, there’s more: they reduce the risk of asthma, colorectal cancer and inflammatory diseases (arthritis, colitis, irritable bowel syndrome, among others), help control hypertension and maintain healthy gums. It’s like having a small army to the care of your health.

Do you know what a whole grain? Quite simply, a grain that retains all its elements. The cereal grain is composed of three main parts: the germ itself, which is the inside or the seed, the farinaceous body that surrounds it, which contains starch, and the peripheral layers (the shell), which contain various minerals and bran, which contains vitamins and fiber to the body.

Speaking of saving, perhaps the best known is wheat, but it actually is part of the shell of the grain, may also be of oats or other grains. Even the rice remains as recommended precisely coverage saved. That’s why nutritionists usually suggest instead of white rice which, by an industrial process, it loses the shell that contains key nutrients.

There is evidence that whole grains are good sources of micronutrients, increase insulin sensitivity and reduce inflammation. In addition, a study by researchers at Beth Israel Deaconess Medical Center in Boston, along with the Health Care System of the city in the United States has determined that men who eat seven or more servings of whole grains per week, were up to 25 percent less likely to have high blood pressure (or hypertension).

Thus, the authors reaffirm the finding that whole grains help maintain heart health because they contain vitamins and minerals like potassium, which can relax blood vessels, and soluble fiber that helps lower the pressure not only but blood sugar levels and blood fats.

Another research conducted in late 2010 by specialists from the Tufts University Research Center for Human Nutrition Department of Agriculture of the United States had found that people who eat whole grains regularly in place of refined grains, accumulate less of one type of fat associated with an increased risk of heart disease and type 2 diabetes.

Cancer Institute in the United States goes further. At the beginning of this 2011 released a report that appeared in the online edition of the February 14 issue of Archives of Internal Medicine, which states that a diet rich in fiber may reduce by 22 percent the risk of dying from heart and respiratory diseases.
For all these reasons, the recommendation is clear: eat more whole grains rich in fiber to ensure a lasting and healthy life. The American Dietetic Association recommends that half the grains consumed are whole day, and ideally, all individuals consume at least 3 servings a day. Do you know how to calculate a portion? Well, a lot may be a slice 100% whole wheat bread, a cup 100% whole grain or ½ cup 100% whole wheat hot cereal, cooked pasta, rice or other grain. Portions may also be calculated by grams: 16 grams equal one serving. Estimated to need, ideally, about 48 grams of whole grains a day.

It is important that you learn to read labels to identify products that are made with whole grains. Keep in mind that food made only with bran are not made with whole grains. If a food is high in fiber, it does not mean a whole grain. Check the list of ingredients: whole grains should appear at the top.

If your package says whole grain (followed by the name of the grain), wheat, integral, integral ground, wild rice, oat leaves, oats (instant or regular), then if it is comprehensive and will be receiving all the nutrients of the grain a whole.
If the package says: wheat flour, semolina, organic flour, multigrain (may describe several whole grains or several refined grains or a mixture of both), then you may be missing parts of some of the grains in it and therefore do not receive all benefits.

If the package says enriched flour, maize germ meal, wheat germ or bran, then DO NOT contain whole grains.
Become an informed consumer so you can protect your health and your family. Best of all is that change can be very simple. To start, simply replace the white rice and bread for their grain equivalent, or add to breakfast a bowl of oatmeal topped with fruit.

Of course, you may have to learn to use these foods in your meals and accustom your palate to new flavors. But if you consider all the benefits you can get, is not it worth the effort?

Cheer up! Nurture your kitchen with new, healthy ingredients, and let your imagination for new and tasty combinations.

There are numerous scientific studies that have attempted to demonstrate the benefits of dietary supplements in a number of situations and pathological conditions. This is a short overview of some of the most important components that have been studied in clinical trials and observational studies:

- Vitamin C or ascorbic acid: studies with megadoses of vitamin C have demonstrated that could have a role in preventing infections. Thus, some studies show its ability to decrease the incidence of catarrhal and pictures of pneumonia. Has also been studied in asthma, no conclusive findings throw. The studies that have explored its antioxidant activity provide data rave about their qualities in the prevention of cancer and cardiovascular disease.

- Folic acid: Folic acid supplementation during the first 6 weeks of pregnancy has been essential to reduce fetal neural tube defects (future central nervous system). On the other hand is attributed a quality studies in cancer prevention. In addition, folic acid prevents cardiovascular disease in people with hyperhomocysteinemia (an amino acid called homocysteine, which increases the risk of excessive arteriosclerosis). An interesting additional data regarding folic acid is that it relates to depression. It is estimated that 35% of depressed patients have a real deficit of folic acid, a figure that rises to increase up to 90% for the elders also found that this deficit seems determined a poorer response to antidepressant drug treatment. It is therefore possible that depression is often a solitary manifestation of folic acid deficiency. Either way has not been shown that folic acid supplements produce a special effect of cognitive improvement or mood in people without previous deficit of this vitamin.

- Vitamin E (alpha tocopherol): From antioxidant such as ascorbic acid or beta-carotene, vitamin E appears to protect against cardiovascular disease and cancer. There are studies that apparently show the relationship between the contribution of high-dose tocopherol and reduced the incidence of prostate cancer. Also recommended for elderly supplement for the prevention of Alzheimer dementia.

- Vitamin D (cholecalciferol): In osteoporosis decreases, combined with calcium intake, the incidence of bone fractures. Has documented possible protective effect for colon cancer, but this should probably be confirmed in future studies.

- Tryptophan: An amino acid precursor of serotonin synthesis (key brain neurotransmitter mood, among other functions) which has been shown to increase brain levels of the substance. There are several studies that report, therefore, its capacity and its apparent antidepressant to boost the pharmacological effect of antidepressants.

- Beta-carotene: An antioxidant that has been attributed the ability to decrease the incidence of cardiovascular disease, especially in smokers, which has not been fully demonstrated.

- Lycopene has been informed of the quality of protection against prostate cancer in some studies.

- Lutein: It is documented in several studies that might be useful for the prevention of AMD, but this has not been clearly demonstrated.

- Omega-3: Clearly amply demonstrated beneficial effect in protecting against cardiovascular disease.

- LCPUFA (free fatty acids, long chain polyunsaturated). Numerous studies with infant formula supplemented with LCPUFA have shown a better development of visual acuity and possibly child’s intelligence level.

- Zinc: Zinc supplementation in pregnancy appear to be related to the decrease in the incidence of babies with low birth weight.

- Selenium: a high-dose parenteral administration, selenium seems to reduce mortality in patients admitted to intensive care units for alleged antioxidant properties.

Dietary supplements are clearly beneficial in deficiency states. The conclusion of their benefit in people without related deficits is very doubtful in most cases, but the relative safety of use means it can be interesting to further explore this possibility in future scientific studies.

The means of supplemental therapy

By definition is a type of therapy that supplements are administered orally, either in the form of capsules, tablets, powders, gel or solution. The basic components are the trace elements supplements (vitamins, minerals and vital substances such as certain fatty acids, etc.).. Dietary supplements are also considered some products that are not properly trace elements but tissue extracts or specific foods (such as shark cartilage.)

Supplemental therapy applications

Dietary supplements therapy is not properly a range of applications on specific diseases. Its applications are found in more varied situations in which it is desirable to prevent a health condition or improve some physical or mental quality. Infections, cancer, cardiovascular diseases, degenerative diseases, etc.. are, to name a few, a good example of an ambitious list of applications that purported advocates say this type of therapy that are possible to prevent. However, it is true that deficiency diseases (such anecdotal so rare in the West) are a clear indication of supplemental therapy, of course. How secure supplemental therapy?

Supplemental therapy is quite safe. There are many people who use dietary supplements regularly and it seems true that involves short-term adverse effects. This is due to the fact that most of the trace elements consumed in excess are immediately eliminated by the body via the kidneys or liver.

But to know that there are some trace elements, by having a greater lipid solubility or metabolic possess different qualities, they tend to accumulate in the liver or adipose tissue of our body, this is the case for example of fat-soluble vitamins, vitamin A, D, E and K, mainly. A group of people very fond of dietary supplements is that of certain elite athletes and people who play sports in gyms, which are consumed ergogenic purposes (to increase muscle strength or exercise tolerance). In this type of people have been isolated cases of liver disease, liver failure with even the horrific abuse taking supplements with soluble substances, but this practice is fortunately exceptional.

On the other hand, has also been seen that some dietary supplements cause drug interactions with certain medications, which deserves to be taken into account people who are subject to certain class of medications as anticonvulsants, antiarrhythmics, etc..

The key factor is a good prevention, which relies heavily on cooperation from the patient. You may need to take medication to prevent clot formation in damaged regions of calcium deposits and cholesterol:

Antiplatelet agents: do not let the blood cells called platelets from clumping together to begin the formation of thrombus, low dose salicylates (eg ADIR), dipyridamole, sulfinpyrazone, ticlopidine.

Anticoagulants act against blood molecules that act in the formation of thrombus, heparin and derivatives of 4-hydroxycoumarin, such as acenocoumarol (best known for Sintrom).Vasodilators may be useful because they increase the caliber of the artery but do not act against the deposition of calcium and cholesterol.

In severe cases, revascularization techniques used, prior imaging study with contrast:

Angioplasty involves inserting a catheter (plastic tube) through the artery to the narrowed area. There is inflated ball that has at its end the catheter, which expands the size of the artery. Then removed it.

Angioplasty with stenting: Once dilated, leaving a hollow tube into the artery to prevent re-narrowing. Can only be done in large arteries such as iliac.

Surgery opens the narrowed area and performs a bridge (by-pass), which replaces that segment by a segment of vein or prosthesis that connects the two healthy ends.

Once revascularized, we must insist on preventive measures and medication is usually required antiplatelet or anticoagulant.

Conclusions

- Atherosclerosis affects not only the limbs but also to the heart through the coronary arteries and carotid arteries to the brain.

- It is therefore very important to the prevention and periodic scans throughout the state of the heart to prevent heart attacks.

- It is believed that snuff and diabetes are major risk factors.

- Currently being tested gene therapy to try to stimulate the growth of new vessels. This process was very useful in cases of severe obstruction of the arteries.

- Snuff: It is essential because existing lesions leave much slow evolution, and the risk is greatly reduced.

- Relatives who have had angina pectoris, myocardial infarction, stroke or stroke, regular checks on the condition of the heart.

- Diabetes: keep serving strictly controlled with medication. Do not raise or lower blood sugar too much and care with medications that contain lactose.

- Obesity or at least excessively high fat diet, try to correct the diet and lose weight. It is convenient control with endocrine doctor or nutritionist, but general recommendations are advised vegetables, fruit and white fish.

- Sedentary lifestyle: making a fitness training under strict medical supervision.

- Hypertension: term compliance to diet and taking medication.

- High blood cholesterol: In addition to the diet and as the case may require specific medication to lower cholesterol levels.

- By performing the medical examination combined with the presence of risk factors and Doppler ultrasound confirmation.

- The appearance of symptoms is characteristic of this disease.

What tests can I order?

Ultrasound can see the narrowing of the artery. If you add the Doppler system can also measure the amount of blood flowing through it. It is not useful for the coronary arteries.

In cases evolved can be injected dye through an arterial catheter and see the caliber of the vessels in the region, along with an x-ray (angiography), by scanning (angioescanner) or MRI (magnetic resonance angiography).

It checks electrocardiogram to assess indirectly the state of the coronary arteries.

It radiographs of the neck, upper extremities, chest and abdomen to detect calcification in your arteries.

What else could simulate it?

- Athletes in the large muscle mass may develop a compartment syndrome of the effort: the continuous physical stress can increase the pressure within the enclosure of the fascia and cause pain. Open the fascia will require surgery.

- When the channel along which the lumbar nerve roots are narrow, you can produce a picture of pain that occurs at a certain perimeter of march, but is accompanied by other neurological disorders and back pain.

- Artery of Buerguer: some young smokers, the snuff produces severe arterial narrowing that can lead to limb amputation. It is mandatory to suppress the snuff to stop the evolution.

- In patients with varicose veins may appear to lie leg pain, but improve standing and walking.

At first the patient complains of cramping, numbness and pain, usually in the calves. It is typical to notice when walking a set distance or “perimeter up” and disappears when standing: vascular intermittent claudication. These symptoms are due to protest of the muscle does not get enough oxygen due to narrowing of the artery.

As the disease progresses, the skin and fat are also affected, appearing ulcers on the heel, ankle and the outside toes, in addition to notice the pain even at rest. The final state is the gangrene, requiring amputation.

When the narrowing is important, a blood clot can completely close the vessel, ordered an acute ischemia: no blood will pass through that artery and the tissues do not irrigate, which manifests itself in a very intense and sudden pain, although there is insensitivity touching the leg, pallor, coldness and lack of pulse. It involves a surgical emergency, should be opened immediately and remove that obstacle.

What does the doctor specific explorations?

- Examine the condition of the skin (color, temperature, ulcers) to determine the outcome of the circulation in the area.

- Perform a test of muscular effort: keep your legs elevated, walk a certain route, etc..

- Palpation of the groin, popliteal fossa of the knee, inside of the ankle and foot back to the doctor tell a decreased pulse, and the placement of the stethoscope in these areas may show a “breath” or turbulence noise blood flow through the narrowed area.

Vascular disorders can affect any artery in the body. Very often appear in the lower limb arteries and are the result of atherosclerosis in the legs and feet.

Born heart the pulmonary artery, which targets the lung and aorta, which is the main artery of the body, which will gradually branching out to carry oxygen-rich blood and nutrients to the body. At its tip iliac bifurcates into branches called that target the pelvis, genitals and lower extremities, forming the femoral artery in the thigh, knee and popliteal several branches to the leg and foot.

The deposition of cholesterol and calcium in the artery wall becomes rigid and less caliber (inner diameter) to pass the blood.

Narrows over time as blood flow that can happen is not enough to irrigate the muscle, which complain with fatigue and pain. In advanced cases pain even at rest and may develop gangrene.
What are the risk factors?

Although it can affect anyone, primarily increases in frequency with age, appearing early as in men.

Risk factors of atherosclerosis are:

- Snuff

- Relatives who have had angina pectoris, myocardial infarction, stroke or stroke.

- Diabetes

- Obesity or at least excessively high fat diet

- Sedentary lifestyle
